Heather Clarke has been overseeing the Care Management & Behavioral Health teams at PA Health & Wellness since inception. Her focus has been on integrating behavioral health, physical health and long-term support services. Heather is a Licensed Clinical Social Worker in both Pennsylvania and California and is a Certified Care Manager. She has worked in the Medicaid/Medicare Managed Care industry for 16 years. In addition, Heather has 15 years’ experience providing direct care services to individuals suffering from severe and persistent mental illness across various levels of care.
